JOB SUMMARY

The Assisted Living Coordinator must exhibit genuine interest in residents and show patience and understanding while working with residents, families and visitors. The Assisted Living Coordinator hires, trains, supervises and schedules our caregivers and med techs. The Assisted Living Coordinator monitors resident care needs, alerts the Wellness Director when the resident has a change in condition, addresses resident and family concerns, and acts as a liaison with community providers. The person holding this position is delegated the responsibility for carrying out the assigned duties and responsibilities in accordance with current existing federal and state regulations and established company policies and procedures.

POSITION OVERVIEW
  • Understanding of standard techniques used in providing personal services for residents and in caring for equipment and supplies as well as the basis of universal precautions to avoid infection of residents and self or contamination of equipment and supplies.
  • Awareness of the assessment process and the development and execution of negotiated care plans.
  • Manage the staffing needs of the Assisted Living Community by interviewing, hiring, training, supervising, and scheduling of care managers and MCM coverage.
  • Assist the Business Office Manager in the completion of new hire paper work, tracking attendance logs, monitoring time sheets, performing employee evaluations per company policy, and staying within the required budget for staffing.
  • Identify staff educational needs including scheduling and coordinating new hire orientation training, weekly mini-modules, and coordinating all mandatory and required in-service meetings for house staff.
  • Ensure that care staff is educated about the needs for our residents and that they are knowledgeable of the resident's negotiated service plan and task/assignment sheets.
  • Report symptoms and changes in condition of residents to the Wellness Director.
  • Maintain accurate records reflecting the resident's conditions, document in the Daily Log, and report all changes to the Wellness Director.
  • Ensure the resident care is being provided by weekly monitoring of all resident's suites and daily walk-through of the assisted living areas.
  • Ensure all infection control guidelines are met and monitor that gloves and protective supplies are available for staff use for resident care.
  • Possess the ability to problem solve effectively and manage various situations according to the MorningStar standards and guidelines.
  • Prepare and administer medications according to procedure.
  • Work with the Wellness Director to develop, maintain, and update resident negotiated service plans and assessments.
  • Schedule and conduct change of condition assessments as well as bi-annual negotiated service plan meetings with residents, families, Executive Director, and Wellness Director.
  • Maintain ongoing communication with the resident and family members regarding the resident's care needs and assist with counseling of residents and families as needed.
  • Responsible for regular resident suite checks and that resident emergency alert systems are answered promptly.
  • Report incidents of suspected resident abuse immediately and complete incident reports within 24 hours.
  • Complete Incident Tracker monthly; work with the Wellness Director on the addendum for interventions and reductions in incidents and review at the QA/safety committee meeting monthly.
  • Assure resident's weights and vital signs are take correctly and recorded at time of move-in, return from hospital or rehab, and monthly.
  • Direct care givers to participate in taking residents to prescribed activities and for those residents who may be physically unable to attend, that they spend additional one-on-one time with them.

QUALIFICATIONS

License/Certification: Successful completion of State approved Medication Administration training and a current first aid certificate as required.

Experience: Minimum of 2 years of health related experience in long-term care or assisted living supervisory or management experience is preferred.

Job Knowledge: Must have the ability to read and interpret documents such as safety rules, operating and maintenance instructions, procurement manuals, and possess strong communication skills.
